How to Create Dewy Forms

• Step 1: Log into your Dewy account
• Step 2: Go to 'Website'
• Step 3: Select 'Create a new form'
• Step 4: Name your form
• Step 5: Select a list you'd like patients to be added to when they complete the form
• Step 6: Drag and drop the fields you'd like to be on the form into the form box
• Create custom fields for questions or topics that aren't listed already ← this will be linked to a Dewy Article
• Step 7: Select 'Integrate'
• Step 8: Copy the embed code and paste it onto your website, or copy the URL to send to patients!

Note for embedded forms: Our forms can be remotely updated, which means that don’t have to re-embed the code for any updates to be reflected on your website.
